My IL-based partnership e-filed 2015 taxes, then changed to an LLC. I understand the IRS still treats the LLC as a partnership, and our business keeps its EIN, but still need to be informed of the name change. Per their website, "Partnerships – Notify the IRS on your current year partnership income tax return (Form 1065). If you’ve already filed your return for the current tax year, you’ll need to write to the IRS at the filing address... For multi-member LLCs, follow the partnership process."
I e-filed using TurboTax Business. What address do I send the notification to? (It doesn't look like we filed with an M-3, and made less than $10 million... does that mean to use the OH address from https://www.irs.gov/uac/where-to-file-your-taxes-for-form-1065)?
Also, what should I send? A blank paper with the old and new business names, and member signature? A copy of the LLC Articles of Organization, adding old business name and member signature?
Finally, the bank we make our deposits to is waiting to change our business name until it receives "documentation reflecting the change from partnership to LLC from the IRS. Once I have confirmation, I will make the name change on the account." Will we receive something like this?

Just write a letter (on business letterhead) to the IRS at the appropriate address noted in the instructions to the 1065 explaining that you changed from a partnership to an LLC in accordance with IL state law. Also note that you will not be making any entity election and will remain taxed as a partnership in accordance with the federal income tax regulations.
Make sure that the letter is signed by a managing member and note that designation below the signature.
It is highly doubtful that the IRS will send you an acknowledgment. However, you can request this in your letter indicating that the bank would like to see a response from the IRS.
Finally, when you file the 2016 tax return, I would also check the box that the name has changed. Just a double check.
