How do I enter W2 box 12 code D if it has prior year contributions? For example: DD $xxxx D 16 $xxx D $xxx

Do not enter the date/year ... just the code D. 

What if including the prior year amount takes you over the annual contribution limit for the current year?

Then do not enter the prior year amount ... it has nothing to do with the current year return anyway.
