Form / evidence needed for IRC 1341?

I am filing a Claim of Right (method 2) per IRC Section 1341 for repayment of (bonus) wages.  Through this forum, I have figured out how to enter it in Schedule 5 (TurboTax allows this is the desktop-version only).  My question is: do I need to provide the IRS any worksheet or form to justify the value claimed?

Note: Turbotax does not allow e-file for this type of adjustment, so I will snail file.  (BTW, this also means I must snail file the state returns, for which TurboTax already collected e-file fees.  sigh.)

The IRS doesn't require any paperwork with your IRC 1341 claim, but since you're paper-filing anyway, it wouldn't be a bad idea to attach your computation sheet to the back of your return.  It could possibly head off an audit.
