Do I fill out boxes 15 and 16 if 17 is left blank?
My employer put in state taxes for Texas but I thought Texas didn't have a state tax. Boxes 15 and 16 are filled out, but 17 was left blank. I've never seen this on a w-2 before.

You are correct, Texas does not have a state income tax. You should leave boxes 15 and 16 blank. The employer is just reporting the state in which the income was earned.
