Box 16 shows two amounts - on top is the same number as in box 1, and below that is shows 0.00. Should I leave it blank, or should I enter the same number as box 1?

This W-2 is for Florida.

If the state in Box 15 is Florida, leave boxes 15-17 all blank to avoid errors.
