Who needs copies of my amended tax forms?

I am sending in my amended tax return. I needed to add form 1098-T that I had received after I filed my taxes. I only have one copy of this form. Do you 1) send this to Federal and State? 2) Who gets the original form?

You can make copies of the form to send and keep the original for yourself.
