Where do I get my form 8962 ...? I followed online instructions to amend return and add 1095A, but when I re-download the updated return, it doesn't include 8962.

Does your 1095-A have amounts in all 3 columns ?  

It has amounts in 3 columns for first 3 months and annual totals... but for rest of the months, the value is 0.00
