What should I put for form 540X part 2 lines 1 through 5? I'm filing an amended 2014 tax return with the addition of form 1095-A.

I'm not sure I'm looking at the same place on Form 540X (Calif, right?) you are looking at. Part II is just simple questions you already know the answers to: your name and address on the original return. If the same, enter "SAME", if not enter your new address; federal determination?: sounds like No for you; your original Calif being audited? Yes or No, you know this answer already; filing an amended federal return for the addition of Form 1095-A? Yes or No, you already know the answer; line 5 is the explanation of changes - "Addition of a Form 1095-A".

Please respond back if I'm looking at the wrong stuff.

Thanks, we are looking at the same page. I'm not clear about line 2 and line 4. Does the letter I got from the IRS asking me to file a 1040x with the addition of details from the 1095-A constitute reporting a 'final federal determination' or an 'amended tax return with the IRS on a similar basis'?

Ah, good. For line 2 that is not a "determination" - that's the end result of an audit. You would *definitely* know if they audited you 🙂 For line 4, you are amending your federal return for the new 1095-A aren't you? If so then that's *Yes*, if not then *No*.

Great, thank you! Do you know if this amended version also needs to be submitted to California, or just the federal govt?

Oh, you know I just reminded myself what a 1095-A is. That's federal only and has no effect on your state return, so no need for a 540X for that, just the 1040X.

Fantastic! One last question because you've been so helpful (can't resist). On the notice I got from the IRS it says I need to file my 1040x with form 8962. But on the turbotax download/pdf I exported (after amending my 2014 taxes with the 1095-A), I don't see anything titled form 8962. Is that something I need to fill out separately, or is it included in-line in the amendment I just made?

The 8962 is the Premium Tax Credit, and if TT/1040 thought it might apply, would have activated it, filled out as much as it could and prompted you for any more entries if necessary. So if you didn't see it in your print out then I would feel confident that it didn't apply to you.

Ok, except the letter from the IRS did say I needed to submit a 8962...
