When you print your return, the federal prints first and then any states are after, in the same document.
- There should be a cover sheet before the Louisiana return which will give instructions for where to mail and which forms to include.
- You should sign and date the bottom of your main form.
- Louisiana Returns and payments are due on or before May 15th.
- I highly recommend to mail it with some form of tracking, so you can prove what date it arrived safely.
- You do not always need to include copies of federal forms in the state filing but there may be some forms where Louisiana wants to see the federal form for various types of income and deductions. If this is the case, they will print with the rest of the state forms and you should include them in your mailing.
- Mail your return to Louisiana Department of Revenue. P.O. Box 3440. Baton Rouge, La 70821-3440.
