What form do I use to request a refund due to IRS error

Out of the blue, I received a form CP504 - notice of intent to sieze (levy) my property. This was the only notice I received about underpaid 2019 taxes. I paid the amount that day but now am digging through my 2019 transcript to detemine the reason for the fee.

 

It looks like the IRS disallowed the IRA qualified savers credit which I do qualify for, based on their charts and my contribution. I filed my return through TurboTax that year and they confirmed I qualify.

 

Question #1 - how do I confirm the reason? I cannot get through their phone system. After 2 1/2 hours on hold, I got disconnected when they closed down the switchboard. Question #2 - what form do I use to request a refund of my payment, which looks to me due to IRS error?

 

Thanks for any help. This is so frustrating.

it's strange. CP504 is a final notice. therefore the IRS is saying it sent you prior notices which you ignored.

you can try the IRS Tax Advocate Service

however since you paid it, your only option might be to file a claim for refund form 843

https://www.irs.gov/pub/irs-pdf/f843.pdf 

did you complete form 8880 in Turbotax.
