The State of WI is requesting a form from me but I can't remember which one it was... where can I find out this info on Turbo Tax?

TTX isn't informed by the state.  So the TTX software won't know what form(s) they might have asked for.

1) Find their original letter that WI sent to you and look at that.

or

2)  Find a phone number to call the WI tax people and ask them.
