Notice of request for information

1. This notice is asking for my W2s... why wasn't that sent in when filing?

2. It's asking for copy of receipts, statements and canceled checks to substantiate each specific medical or dental expense. Do I need the copy of receipts and statements and caceled checks or will one suffice?

The IRS is sent the info you entered on the return ... the actual W-2 form is sent by the employer to the SS administration who usually posts the info to the IRS.  

 

So the IRS has requested that you provide information they listed ... follow the instructions to the letter.

 

The medical expenses ... only send copies (never originals) showing you incurred an expense and paid it.  If you can get a copy of the paid statement from the health care providers and the pharmacy that may be easiest ...   see if  you can get an annual statement from them.  Remember if you cannot prove the expenses and payments they will deny the deduction.
