No document number or issued date on NYS interim license. Should I use info from the old one? Haven't received new plastic one yet after renewing in March.

Your temporary license should have the information needed on it. However if it does not, and your physical license is not expired you can use the information on the physical older license. 

There isn't a license number on it (above your name and address)?
