Just discovered a mistake on my W2; the boxes for state 15,16,17 are blank, but should be filled in. I'm at the filing deadline. How should I deal with this?

I work in a different state than my home state. On my W2, Boxes 15, 16, 17 contain data for the state I work in, but this year I discovered the boxes are blank for my home state. In the past, these boxes were filled in; so, it's a mistake by the W2 issuer. I need to file state income taxes where I live. I am at the filing deadline, so I plan to request a corrected W2, but most likely cannot get it right away.  So, should I file an extension for federal and the two states, and then file amendments once I receive the corrected W2? Or, should I fill in the correct state info into turbotax, and go ahead and file all the returns now? (I know the data that should be in those boxes).

Not a tax expert, but I would pay my estimated taxes - if you have to pay - and file for an extension. Then call your employer to work it out.
