I was just wondering how I would add Form OWCP-957 to my taxes when filing?

I was injured on the job last year and I went to physical therapy and several doctors appointments

You won't need to include Form OWCP-957 on your tax return. This is a payment made to pay for medical travel costs related to a work injury or illness. Since these are payments for personal injury or sickness under the Federal Employees' Compensation Act (FECA), they aren't taxed.
