I want to record a bankruptcy loss on a stock investment, should I use Form 1099-B?

I have to file my 2020 tax returns by 10/15/2021 and I want to include a stock investment bankruptcy. Should I file on Form 1099-B?

Have your broker declare the stock worthless and report it on the 1099-B as sold for zero ... then you can enter it on the return.
