I received my W-2 from the IRS and it does not show the state tax withheld?

My past employer refused to send out W-2s so I received ..something from the IRS that is barely legible and does not show what state tax was withheld. I am at a loss of where to find this information at this point.

The IRS never retains the state information portion of the W2.  You should use your pay stubs if at all possible to verify the amount of state withholding.  If, by any chance, your state is one that withholds a flat rate of tax, you may be able to use that amount.  If not, the burden of proof will be on you for the correct numbers.
