I received a form from the IRS stating that I got paid "interest paid or credited"which I actually received. the form has no number. how can I file this?

The IRS is informing that I have to file this as an income

You report this as interest income on Form 1099-INT.  You enter the IRS as Received from and then enter the amount in box 1 of the 1099-INT.
