I received 1 document from turbotax after I filed my state and federal taxes. Which forms should I send to each place? In order, I received a 1040, 540, and another 1040.

The first 1040 pages go to the IRS.  The 540 is the California return and they want a copy of your federal 1040.  There should be an instruction sheet in front of the federal 1040 and one in front of the CA 540 return.
