I live in the state of California. Does anyone know what Notice FTB 4522 EDR was sent by mail?? What is this notice?

This form means you went online to the FTB and requested the PIN to create a new account.  They mailed the PIN and that "form" (really a letter) is FTB 4522 EDR.  I panicked when I saw it too!
