I have no Idea what the 3506 Provider Identifying #-1is

California requires that you enter the care provider's SSN or ITIN in order to claim the child and dependent care credit, if the provider was an individual person.  If the provider was not an individual, the FEIN (federal employer identification number) is required.

You'll have to obtain that information from your care provider.
