I had to amend my taxes but I haven't received my refund yet. Do I send in the 1040 form or just the 1040X form? Will I still receive my original refund from the 1040?

Yes, you'll receive your original refund. If you're amending to claim more money, the IRS says to wait until you've received your original refund before filing an amendment. 

You would mail in form 1040X and the new form 1040 and all the schedules. When printing your tax return(s), select the Forms required for filing option. TurboTax will print out what you need to send in.

Once you've received your original refund, feel free to cash or spend it while your amendment gets processed (this can take 12–16 weeks).

Three weeks after you mailed your amendment, you can start tracking its progress at the IRS Where's My Amended Return? site.

Is the new form 1040 the original 1040 that I already completed on Turbo Tax?

It is the new one that is created when you amended.
