I got a letter requesting more heath care info and then I also received the 1095-b form in the mail but I lost the paper that tells me where to send it to ?

They won't send by fed return until they get this documentation , I don't know where it needs to go.

The IRS web site explains more about form letter 0012C requesting more information about the 1095-A. That web site gives you a number to call (1-866-682-7451, extension 568) for more information, like the fax number or address to send your form.
