I am trying to amend my 2021 tax return because I received several different tax forms in the mail after I filed but those form names aren’t listed as options for amend

5498-SA and 1095-C

Good news ---you do not need to amend for either one of those.  Nether one of them even needs to be entered.

You do not enter a 1095C on a tax return.   You would only need to enter a 1095A if you had marketplace insurance.  A 1095C is not for marketplace insurance.

Well that’s good news! Thank you! I guess I just assumed because it was mailed to me and is a tax form that I needed to include it. 

Just to confirm- I do need to amend for two 1095-SA forms I missed, right?
