I am amending my taxes for a previous year and need to submit a 1099-SA form and a 5498 form; where do I do this?

A form 5498 is an informational form only and is not entered on a tax return.

 

For the Form1099-SA -

Click on Federal Taxes (Personal using Home & Business)

Click on Deductions & Credits

Click on I'll choose what I work on (if shown)

Scroll down to Medical

On HSA, MSA Contributions, click on the start or update button
