Form 564-19 from the state of Oklahoma is:
A $5,000 credit is allowed to a qualified employee who has a degree in a qualified program and who, on or after January 1, 2009, is employed in Oklahoma by or contracting in Oklahoma with a qualified employer whose principal business activity is in the aerospace sector. The credit is allowed each year for up to the first five years. To qualify for the credit the employee may not have been employed in the aerospace sector in Oklahoma immediately preceding employment or contracting with a qualified employer.
When you are in your state return, you will get to a screen titled Tax Credits. Click Start next to Credit for Employees in the Aerospace Sector and answer the questions in the screens that follow. On the next screen, be sure to click on the CLICK HERE link for the information or see the form in the link below.
I have the form filled out and on my computer, but Turbotax doesn't ask for it, and when I go to the Forms view, it is not listed as one of the forms in my state tax return. Do I need to physically file so I can just add the form into the ones generated by TurboTax? Is there no way to add an additional form as a PDF so that I can just electronically file?
You will have to mail the form in to your state.

I know this is an old discussion thread, but I had a question come up related to this. My spouse and I got married in late 2022 and both receive the Oklahoma Aerospace Tax Credit. Now when filing jointly, how do we both submit a form? TurboTax only seems to allow one form to be entered, but we both have a form to enter. Any help would be greatly appreciated!
