How do I get a AR-1000-DD form that is completely filled out from my 2023 tax return?

I received a letter from the state of Arkansas saying I needed this form along with proof of my son's Intellectual disability to claim credit for a Developmentally Disabled dependent credit

Form AR1000-DD, Certificate for Individuals for Developmental Disabilities, is not available in TurboTax. You would need to complete that certificate and then have a Physician complete a portion of it and sign it. Here is a link to the form that you can complete online:   Form AR1000-DD
