This would be Form 4562 Depreciation and Amortization Report in your records. 

That form is not in the PDF that Turbo Tax saved.

You have to save all worksheets when you save your file. If you didn't save the tax file that way, locate the actual Form 4562 in your pdf. @loweyecue
