Look for ISS on your license. That is the issue date.

States requesting the driver's license is an identify theft precaution. The state that I live in specifically says that information from your driver's license is not required.
However, I would expect not providing the information or providing information which the state may not readily be able to verify (like a temporary driver's license), may delay processing of your state tax return.
