Federal Payment Error - No account/unable to locate account - State Refund worked just fine with identical bank information

Federal and State taxes were submitted at the same time. For Federal, money was owed and direct debit bank information was provided. For State, a refund was due and same banking information was used for the refund. Th banking information matches identically.  The State refund came through just fine. For the Federal tax due, an error message was noted on the IRS Payment Activity screen that "no account/unable to locate account". Subsequent late filing penalties and interest have been assessed.

Any thoughts on why a payment would be rejected and a refund went through just fine to the exact same bank account?

What bank?  Some banks use different routing numbers for payments and deposits.  Ask the bank.

Sallie Mae Bank.

Apparently the IRS only sends through account information and NOT name information when requesting funds to be withdrawn for tax payment. Sallie Mae Bank requires name to be included in the requesting transaction. If name is not included, the requesting transaction (and subsequent account) is flagged as not being found.

Wish either TurboTax or Sallie Mae Bank would make taxpayers aware of this so they aren't charged late fees and interest penalties by the IRS. Very frustrating.
