Do I send in the additional information page, the last page printed up, along with the rest of my return?

The Instruction page will advise exactly what Forms you would need to mail in along with your Return.  The Instruction page is a part of your Return printouts.  So use your instructions as a guide as to what to send in with your completed Return.
