Do I answer yes or no on the 1099g - question: Is the "corrected" box checked? Is it a circle to the left a number 5 in it? What does it represent?

The check box for the 1099-G Corrected form is at the top of the page next to VOID.  If this is your first 1099-G from that location it is probably not a corrected form.
