Bank that has my HSA sent the IRS 2 disbursements instead one one

We received notice of an error from the IRS. with our return due to only listing one HSA disbursement. The IRS showed 2 disbursements. The bank thinks it was an IT error. There was only one disbursement from the HSA. They want to provide a letter explaining that. What should be it that letter?

First have the HSA issue a corrected form to negate the erroneous one issued  and then simply explain it to the IRS in simple terms and attach any supporting documents that prove your position.
