Amending from Married Jointly to Head of Household

Will I need to provide documents as proof that my husband and I do NOT reside at the same address or is there anything else I need to submit once I amend?

You do not need to submit any documentation to prove that you do not reside with your husband with your amended return.  If the IRS requires any proof or documentation, you will be contacted by mail requesting the information.
