Widower filing out a w-4p form

I am trying to help my mother out with filing out the W-4P form for my recently passed Father's military pension. 

 

Here's where it gets tricky. My mom has been medically retired since around 2008 so she receives disability social security. My dad did 25 years in the military but also did 36+ years at the city. So both government jobs. My dad fully retired in 2021 and he recently passed in October 2024. Also since my brother and I are adults, she has no dependants.

 

The form the military sent us--W-4p-- would my mom only put down his lowest receiving pension in box (bii) and nothing of hers since she receives disability social security?
