Where is my distribution from

I withdrew money from non qualified annuity and the where is my distribution from screen what box do I check 

The annuity is from a financial institute

You should have received a form showing this withdrawal.  If it was reported on a 1099-R then you'll use that section to input it into TurboTax.  If you received a different form then you should enter it directly from that form.
