Transmission didn't go through: error message difficulty recognizing data you are sending. Duplicate attach name detected '/attachment//DEStateReturn.pdf' Please advise

I've gone through and completed the DE state tax return a second time and tried E-filing all other states/Fed return while selecting file later for DE and still received this same message.  Not sure how/where to make the correction.
