Was this received as an email? Were you expecting a Form 1099?
This could be a scam.
If received as an email and you do not recognized the email address of the sender then delete the email and the attached form.
I can't recall the issuer. It's been sitting in a folder on my computer for a couple months. I know the sender is legit and I had to open one of these before that was legit. How do I identify the sender if I don't have an email or link to figure out who sent it?

I don't recall whether it came by email or download. It could be from a payer I worked for, or a payer such as an account I hold.

How about the process of elimination? Did you work for a payer or have an account with a payer from whom you did not yet receive a 1099?
