RMD from TSA 403 (b)

Took a distribution last year from this annuity but only received a letter showing the breakdown of my gross, tax withholding, and distribution.  Should I have received a 1099 R instead?  How do I enter this into TT as there is no EIN for the company that issued it?  The letter was issued from Midland National Life Insurance Annuity Division and is not part of my normal retirement from the State.

Yes, you should have received a 1099-R to report your distribution from your 403(b).

 

You may be able to access this tax document from the Payer's website, or get the EIN number to enter a Substitute 1099-R.
