All of them. All 5 will issue a K-1 to the master LLC which the master will report on that return. Then the master will issue you a K-1 which you will report on your personal tax return.
This is assuming ALL LLCs are partnerships and/or have elected to be treated "like an S-Corp" for tax purposes only.
