How to fill out form 8853?

I put money into the HSA but do not have HDHP. In 2017 I had HDHP. For 2018 I enrolled in my wife's health insurance and it was not an HDHP.  I contributed to the HSA.  How do I answer the the questions on the smart sheet and complete the 8853 form with the accurate information?

I just want to access the form.  Why can't turbo tax allow me to do it.  This is crazy.  I've  never before had this much trouble with turbo tax in all of the years I have used it.  I just need a simple answer.  This is the worst experience I've ever had with you!!!!!

What are you trying to enter?  A MSA, HSA, HDHP or a Long Term Care Insurance Contract?
