if there was an foreign real estate inherited in 2023, does the 3520 need to be filed ever year starting now in 2024 or only one time?

You do not need to file every year unless the real estate was placed into a trust and if you are the owner of the trust. If you are the trust owner, you do have an annual reporting  requirement.
