Filing an amended return

I have to file a federal amendment, a CA state amendment, a NY state amendment, and a NC state amendment. I have a couple questions about this process:

1. When I mail my state amendment do I have to include a copy of my federal amendment?

2. How should I secure the documents? Can I use a paper clip, stapler? Are there any restrictions I should worry about?

Requirements vary by state but you can’t go wrong by including a copy of your federal amendment along with the forms that have changed because of the amendment. 
A stapler would be fine.
