We need to file an extension for a child (under 18) who hasn't filed taxes before. If I understand correctly, the first time I file a child's tax return, I have to mail it in. Does that also mean that I need to mail in an extension?

Yes, you will have to print and mail the extension because you won't be able to provide the prior year's adjusted gross income (AGI) for verification.
