Chase 1099-INT For Bonuses

Hi..I am having trouble entering this Chase 1099-INT that we got for opening a checking & savings account. There was a $400 bonus for the checking and a $200 bonus (plus minor interest) for the savings account. I go through the step for manually entering it but have problems starting at box #14 and I don't know (or see) the tax id number for Arizona. I guess just boxes 14 thru 17 are confusing me.

 

If anyone has any advise how I enter this as a 1099-INT it would be greatly appreciated.

Not all of the boxes on a Form 1099 INT will have entries and since your box 14 to 17 say see details, and there are no details below, except that box 15 is AZ, you may leave these boxes blank.

the interest/bonus is fully taxable for federal and state purposes. so the only entries needed are the payer's name and amount which is entered in box 1 of the 1099-INT
