1099-R Help

I need help with to me is complicated question.  I am 58 yo.  In 2018 I rolled 1/3 of my 401k into prime variable annuities with an Insurance company to help pensionize this money for retirement.  The Fidelity funds were distributed in 2 distinct payments and I received 2 1099-R.  The larger of the sums was directly rolled into traditional IRA and had code G in box 7 on 1099 R.  The smaller of the sums was my contribution, with the same numbers in box 1 (gross distribution) and 5 (Employee contribution) as this was the $$ I had contributed over time.  My distribution code for this is 1.  The complete sum of this second transaction was contributed into the annuity.  I am able to choose proper buttons for Federal Tax but I am PA resident.  I have already paid PA state tax on my contribution as 401k is not pretax deduction in PA.  I need help in properly preparing PA tax form as have already paid state income tax on this sum.
