W-2 Missing a State and State ID

I got a W-2 from Omaha Life Insurance for short term disability when I was on maternity leave. Unfortunetly they didnt take state taxes out of it and therefore theW-2 has nothing in boxes 15 for Sate and Employer State ID Number but there is an amount in box 16. TurboTax requires that I provide data in the two boxes for 15 so what should I enter here? Or should i just not enter anything in boxes 15 and 16?

If there was no state tax withheld in Box 17, leave Boxes 15-17 blank.  This will avoid errors in your return.

Thank you! Okay so just to confirm leave boxes 15-17 empty, even though my w-2 has box 16 populated?

Yes, that is correct. Your state return will still include this as income.  But without leaving boxes 15-17 blank, it will be impossible to e-file.
