Upload WI Form 804

My Wisconsin return rejected as it does not have Form 804 attached, which I have uploaded to TurboTax.  Apparently it did not attach to the return, as TurboTax indicated it would.  Please advise next step to fix this problem so I can file electronically.

With TurboTax you can't attach a PDF to an electronic filling. Instead, you will need to send your state return by mail and attach a physical copy of your Form 804. 

 

How do I file my return by mail?
