CA Form 3514 for Clergy

How do I handle form 3514 as Clergy when I am eligible for the EITC of CA, but not sure what information to enter for a business name, address, zip, and code[813000]? Is this information necessary as I am a pastor, but not the owner of the church?

Use your own name as business name and address for the CA 3514- if you do not have a business license number or SEIN, leave blank.

Can you clarify for me.  I am a pastor and don't have a business license.  Would I leave this blank or put my name & home adress?

Yes. Put your name and home address if this is a Form Schedule C.  Leave blank any information that you do not have or does not apply to you.
