What do I put down for line 14 column (e) on my 1099-B form?

My paperwork does not show line 14 on 1099-B form. Where do I get this information from?

Box 14 of the 1099-B is State Name.  This is the two letter abbreviation that represents your state.

 

Box 15 is State Identification.  If this is not available you can use the Payers TIN.

Box 16 is State Tax Withheld.

Your work around currently for states that is not working for states that don't have state tax. the 1099-B box 14,15,16 are not required if box 16 has a zero (0). THen you have delete box 14,15,16. The IRS (John Smith) has said that box 14,15,16 should be filled in with state, TIN and amount. Even if the amount is 0.

 

YOUR system has kicked back 207 of my 1099-B because for box 16 has a zero (0). Since Florida doesn't pay state tax, we are required my the IRS to have 14,15,16 filled in correctly.

 

This is my 19th work around. Hopefully I am covered if I'm audited by the IRS.
